The Heart Failure Out-patient Clinic at Blythe House is part of the Derbyshire Heart Failure Nurse Service. This service provides support to people living with heart failure, both at home and in out-patient clinics.

Fortnightly Out-Patient Clinic at Blythe House

The Heart Failure Out-patient Clinic at Blythe House takes place on a fortnightly basis. This is usually the second and fourth Monday of the month. The specialist nurse taking the clinic helps patients to manage the symptoms of heart failure and learn self-care strategies. She also provides links to other services that can help, such as Cardiac Rehabilitation and Social Services.

How to Refer

Anyone in the local area who has a confirmed diagnosis of heart failure can be referred to the clinic via their GP, community matron or practice nurse, or following a hospital admission. Living Well Service patients with heart failure can be referred by their key worker.

Note: Professionals referring their patients should please note that a heart failure diagnosis must be confirmed by echocardiogram prior to referral.
