Who is the course for?
Any patient who has a diagnosis of a life limiting illness, such as COPD, Heart Failure, Cancer or a Neurological condition and has two of the below symptoms:
To be referred to our services you must be a resident or registered with a GP in the High Peak or Derbyshire Dales.

We have expert nurses, physios and occupational therapists to show you the best ways to control and manage your symptoms.

By the end of the 8 weeks course we aim to give you a choice of self-help strategies to help you with these symptoms and improve how you feel.
When is it?
The eight-week FAB programme runs from 1.30pm to 3.30pm every Tuesday.

How can I be referred?
Any health care professional can refer you or you can fill out your own self referral form. Simply visit our Blythe House website to complete a referral form or email us at bhhreferrals@nhs.net.
What happens once I have been referred?
We will contact you via telephone to discuss your referral and book you onto the next available course.
